1. Check fit

A short eligibility review confirms that your needs match what the practice offers and that you will be located in a state where the practice is seeing patients. It covers practical questions such as location, Medicare status and the reason for your visit. It is not a clinical visit.

At opening, appointments are planned for patients located in Arizona. Dr. Khan is licensed in Arizona, Utah, Michigan, Ohio and Illinois; other states will be added as each is ready.

2. Complete secure intake

Before your first visit you will receive a link to a secure patient portal to complete forms, questionnaires, consent and payment details. Medical information is collected only through that portal, never through email or this website.

3. Attend your Clarity Evaluation

The Comprehensive Psychiatric Clarity Evaluation is a video visit with Dr. Khan, typically 75 to 90 minutes. Join from a private, quiet space with a stable connection.

Your location matters. At the start of every visit you will be asked to confirm where you are physically located. Visits can only take place when you are in a state where the practice is seeing patients. A billing address or phone number does not substitute for your actual location.

4. Review your plan

Dr. Khan explains his formulation, the diagnoses he considered, and your options. You decide on next steps together. With your permission, he can coordinate with your therapist or primary care clinician.

5. Follow-up

If you continue care, follow-up visits keep the plan current. Some patients instead return to their existing clinicians with written recommendations. Either path is fine.
